Patient and Family Advisory Committee (PFAC)

Mosaic Primary Care Network (PCN) currently hosts a Patient and Family Advisory Committee (PFAC). The committee is looking for additional volunteers that are interested in becoming a Patient Advisor, to join the Committee in September 2022. The role of patient advisors is to be the voice of patients and families making a difference on the health services provided in your community.

 

Who can be a Patient Advisor?

Anyone who is a Mosaic Primary Care Network (PCN) patient, over the age of 18, and willing to volunteer for two hours per month. These monthly meetings are hosted online with other members of the committee to help improve the safety and quality of care for all patients of Mosaic PCN!
